I had to check this. I found two (or actually three) occurrences of the name: in chapters Elrond's Council (2004 and 2005 editions: p. 244; 1981 (4th) edition, paperback: p. I/320) and (twice) Homeward Bound (2004 and 2005 editions: p. 993; 1981 edition: p. III/331); the name is always Deadmen's Dike. Same in the Indexes of 2004 (p. 1150) and 2005 (P. 1150, 1156) editions. Also Robert Foster (The Complete Guide to Middle-earth, paperback edition 1993) has Deadmen's Dike (p. 83).
